That’s why we’ve established our Enhanced Care Management (ECM) Program—to serve as the guiding hand through these complex processes and provide the comprehensive care and support our community deserves.

To Affordability:
For CenCal members who qualify, our services are completely free of charge. We’ll support you every step of the way, making the enrollment process easy and effortless. We pride ourselves on our extensive experience and ability to efficiently guide you through the process of receiving the excellent care you deserve.

What We Offer

Dedicated Care Managers: Each member is assigned a Lead Care Manager who serves as the primary point of contact, ensuring consistent and personalized support. Our care managers are passionate advocates who coordinate with healthcare providers, social services, and family members.

Personalized Care Coordination: Our ECM program offers individualized care planning, connecting members with the medical, social, and support services they require. With a focus on preventative care and early intervention, we aim to enhance well-being and prevent emergency health situations.

Community Outreach and Engagement: We believe healthcare should be accessible to everyone. Our outreach team tirelessly works to engage with community members in both urban and rural areas, establishing trust and making sure that those in need are informed about and have access to our services.

Multilingual Support: Language should never be a barrier to receiving healthcare. Our staff includes bilingual professionals who can communicate effectively in English and Spanish, ensuring that we meet the cultural and language preferences of our diverse community.
