Notice of Privacy Practices

Notice of Privacy Practices

The Wisdom Center | Wisdom Adult Day Partners, LLC
Effective September 28, 2026

This notice explains how health information about you may be used or shared, how you can obtain it, and your privacy rights. Please read it carefully.

Who this notice covers

This notice applies to Wisdom Adult Day Partners, LLC, operating as The Wisdom Center, including our adult day health care/CBAS, Enhanced Care Management, and related services. It applies to health information we create or receive in providing or arranging services, including information submitted through a secure website inquiry or referral form. Sending an inquiry does not enroll anyone in a program.

Your rights

See or obtain your records. You may ask to inspect or receive an electronic or paper copy of the health information we maintain about you. We will respond within the time required by applicable law. We may charge a fee only when permitted by law; some copies must be provided without charge.

Ask for a correction. If you believe information in your record is incorrect or incomplete, you may ask us to amend it. If we deny the request, we will explain why in writing and tell you how to submit a statement of disagreement.

Choose how we contact you. You may ask us to communicate with you at a different phone number or mailing address, or by another reasonable method. We will accommodate reasonable requests.

Request limits. You may ask us to restrict certain uses or disclosures for treatment, payment, or our operations. We are not always required to agree. If you pay us in full out of pocket for an item or service and ask us not to disclose it to your health plan for payment or operations, we will honor that request unless a law requires disclosure.

Get an accounting of disclosures. You may ask for a list of certain disclosures we made in the six years before your request. The list does not include every disclosure, such as most disclosures for treatment, payment, and health care operations. One accounting in a 12-month period is free; we may charge a permitted cost-based fee for additional accountings.

Receive this notice and choose a representative. You may ask for a paper copy of this notice at any time. A person with legal authority to act for you may exercise your rights; we will verify that authority before acting.

Complain without retaliation. You may complain to us or to the U.S. Department of Health and Human Services if you believe your privacy rights were violated. We will not retaliate against you.

How we usually use or share health information

Treatment and care coordination. We may use information to provide services and coordinate with your clinicians, health plan, and others involved in your care. For example, we may share relevant information with a physician or CenCal Health care team when coordinating a care plan.

Payment. We may use and share information to determine coverage, obtain authorizations, submit claims, or collect payment from CenCal Health, the Department of Veterans Affairs when applicable, other payers, or you.

Health care operations. We may use and share information to manage our programs, review quality, train staff, address complaints, meet contractual duties, and contact you about your care or our services.

Service providers. We may share information with vendors that perform work for us, such as secure form, email, record, or billing services, when permitted by law and subject to required safeguards and agreements.

Other uses or disclosures allowed or required by law

Subject to applicable federal and California limits, we may use or share health information for public health activities; reporting abuse or neglect; health oversight and audits; research when legal requirements are met; workers’ compensation; organ or tissue donation; coroners and medical examiners; or to prevent or lessen a serious threat to health or safety. We may disclose information when a law requires it or in response to a legally valid court order, subpoena, law enforcement request, or other government request. Special protections may limit disclosure in particular circumstances.

Your choices and written permission

You may tell us whether to share relevant information with family members, caregivers, or others involved in your care or payment, or with disaster relief organizations. If you cannot tell us your preference, we may share information when permitted by law and when we believe it is in your best interest.

We generally need your written authorization before using or sharing your information for marketing, selling your information, or sharing psychotherapy notes for purposes requiring authorization. If you authorize a use or disclosure, you may revoke that authorization in writing; revocation does not undo actions already taken in reliance on it. If we ever use health information for fundraising communications, you may ask us to stop.

Substance use disorder records and California protections

Some substance use disorder treatment records are protected by 42 CFR Part 2. If we receive or maintain these records, we will follow the applicable special rules. In particular, such records generally may not be used or disclosed in civil, criminal, administrative, or legislative investigations or proceedings against you without your written consent or a qualifying court order and subpoena or other legal mandate. If fundraising would use Part 2 information, we will give any advance notice and choice required by law.

California law may provide additional protections for medical, mental health, substance use disorder, reproductive health, or other sensitive information. We will follow the law that provides greater protection when it applies.

Our responsibilities

  • We must protect the privacy and security of health information covered by law and follow the practices described in this notice.
  • We will notify you as required by law if a breach may have compromised your information.
  • We will not use or share your information outside the circumstances described here unless you give valid written authorization or the law otherwise permits or requires it.
  • We may update this notice. A revised notice will apply to information we already maintain and to information received later. The current version will be available at our office and on our website.
